Shipton-on-Cherwell train crash

The 1874 Shipton-on-Cherwell train crash in Oxford, UK was not related to brake failure as it says in the sub-heading under Incidents. Perhaps it might be best to revise "...involving defective or improperly-set railway brakes..." to "...involving equipment malfunctions..."

On the other hand, The 1874 Shipton-on-Cherwell train crash seems to belong on the Train wreck Wikipedia page. It doesn't seem like this was a Runaway train.
