The second term in the denominator needs to be either [cumulative Gain(n-1)/(input)P1dB(n)] or [cumulative Gain(n)/(output)P1dB(n)]. This can be confirmed by using reference 17 and translating it to the input equivalent.
This is more easily derived from the TOI/IP3 equations. The third order equivalent is [cumulative Gain(n-1)/(input)TOI(n)] or [cumulative Gain(n)/(output)TOI(n)]
