Summary Edits/Added Prevention Methods

Hi,

I have edited the summary paragraph to read a little more smoothly and changed the sentence summarizing how they occur to a more general cause.

I also added a section talking about how to prevent flow lines with the sub sections organized by prevention method type. Most of the material written was from personal experience, as I used to work in an injection molding factory,but I did add sources to back up the content written.
