Hashim Ali Zaibak (born July 15th, 1979, Gaza, Palestine) is a Palestinian American pharmacist and entrepreneur. He is the founder and CEO of Hayat Pharmacy, a Milwaukee based independent pharmacy recognized for its community-focused healthcare services, multilingual patient care programs, and public health initiatives.
Early Life and Education
[edit]Hashim Ali Zaibak was born July 15th, 1979 in Gaza, Palestine. He is the child of Palestinian refugees. Zaibak immigrated to the United States in 1992 and settled with extended family in Chicago, Illinois.[1] Zaibak completed his Associate of Science degree at community college at Moraine Valley Community College in 1996. He subsequently attended pharmacy school at the University of Illinois-Chicago from 1996 to and earned his Doctorate of Pharmacy (PharmD) in 1999.[2]
Zaibak has also obtained certifications in diabetes education, immunization services, medication therapy management, and professional communication.[3]
Professional Career
[edit]Following graduation, Zaibak moved to Milwaukee for work at Osco Pharmacy, which later transitioned to CVA pharmacy in 2006.[1] Throughout his time at CVS, he advanced into management positions and served as a district pharmacy supervisor overseeing multiple pharmacy locations across Wisconsin.
Zaibak founded Hayat Pharmacy in 2011.[4] The name "Hayat" comes from the Arabic word for "life."[5] Zaibak currently owns and operates 18 pharmacy locations that provide services such as medication therapy management, prescription delivery, medication synchronization, in-home care, and community outreach programs.[6]
Zaibak has served as a clinical instructor since 2019 for training pharmacy students across multiple pharmacy schools in the states of Wisconsin and Illinois.
Public Health Initiatives
[edit]Zaibak is an advocate for healthcare access in underserved communities. Through Hayat Pharmacy, he has developed programs focused on medication adherence, multilingual patient services, home delivery, and outreach to vulnerable populations.
During the COVID-19 pandemic, Zaibak played a role in testing, vaccine distribution, and public education efforts in Wisconsin. Hayat Pharmacy was among the first independent pharmacies selected to administer COVID-19 vaccines.[7]
Personal Life
[edit]Zaibak is married to Bushra Zaibak. They have three children together.[8]
Awards and Recognition
[edit]Zaibak was named Pharmacist of the year in 2014 by Pharmacy Development Services. In 2021, Zaibak received the Bowl of Hygeia Community Service Award.[3]
In both 2014 and 2021, Hayat Pharmacy was named Health Mart's Pharmacy of the Year.
