See Template:Small/doc ... Note 85% instead of <small>. This template uses css "smaller", which, like <small>, is not the same in all browsers, and needs to be 85% like {{small}}.

Change both occurrences of:

font-size: smaller;

To:

font-size: 85%;

Converted to columns

I've converted this template to use column notation ({{col-begin}} etc) instead of table notation. Unfortunately table notation causes the page to be unreadable on small screens as the table cells become too narrow. Columns are instead re-flowed into vertical sections. I like the idea of the column tweaks.
I'm going to revert the coloring of the headers, because the red and purple text confused me into thinking they were links (to nonexistent and visited pages, respectively).

Please fix default for no image parameter value

Image width parameter not working
