Usage

Ideally, the timestamp would be ~~~~~, but if not, copy and paste the timestamp from someone's signature. It'll also work fine without it.
If you're in a hurry, use this:
{{rfcu box|{{subst:SUBPAGENAME}}}}
Note: You must subst SUBPAGENAME, or else the page won't work properly when transcluded. This error is sneaky, so be watchful.
With clerknote, it is an optional field, and should only be used for special circumstances related to an open case; for example, if it is deferred to another checkuser, or more information is requested by a checkuser. The field should be removed once the issue in the clerknote field is resolved, and/or the case is dealt with (either declined or completed).
The template also adds __NOINDEX__ to each page it is transcluded onto (i.e., every RFCU subpage).
