I apologise for the patchy nature of this item. It was jotted down a long time ago, not updated, and I have little time to flesh it out at present. I've noted the concerns of other editors and begun the work of tying it to references and relevant debates. There's more to do here, and if no-one else picks it up I'll try to do that soonish.
