Neither Sen, not Sharma.
[edit]On my website, I just posted an image from filmindia of 1936, clearly Sen; see here
However,
I was able to talk to his son, Kailash Surendranath. He said that his father never used any last name. He confirmed that his grandfather was Pundit Ralia Ram Sharma but none of the next generation used this last name. So, I am removing Sen and linking the son's Wikipedia page to it.
I am hoping to persuade him to write a book on his dad, so that we can get genuine info,
