Ritual for installation of a new bishop

In the ritual for the installation of a new bishop, he is supposed to approach the closed doors of the cathedral and knock to request entry. I believe, these days, that he knocks with his crozier or something. Was the sanctuary knocker an integral part of this ritual in times past? It seems strange to me that an exceedingly rare usage (requesting the right of asylum) was the primary or most well-known usage for these prominent articles.
