I have re-written the article from my own knowledge, without following my text book anywhere nearly as closely. Sorry for the inconveniance.

Incidence

The source I used for increased incidence in central Appalachia suggests increased surface mining producing more silica dust might be responsible. Also increased sorting of coal and stone.
