Cleanup started

I've done quite a bit of work on this to make the English more idiomatic. In one or two places I had to use intelligent guesswork as to the exact meaning, especially in the case of the quote from the psychiatrist, so I haven't yet removed the cleanup tag - hopefully it can be checked by someone whose French is better than mine.
