Third Edition

There is a third edition of the book, with one or two new chapters and a new preface. This should be mentioned but I didn't add it myself, first because I wasn't sure what to cite or how to do it, and second because there is some slight confusion regarding publisher and year of publication. It was published by Cambridge University Press in January 2006, but the copyright page says "First published in print format 2005" and Columbia University Press is mentioned somewhere as a publisher.
