Loss of top octave?

The section on his run-in with Charles Mingus says that he lost his top octave "...for almost two years". The article on Charles Mingus says that he suffered "permanent loss of the top octave". There is no reference given in this article, and the reference given in the Mingus article just says that he "lost an octave of range on the trombone". Does anyone know the details?
